Below, you can download the Greater Boston Real Estate Board Rental Applications and Proper Realty Cosigner forms for your new apartment.  Please note the following for each of the forms.

Rental Applications – Download

• Write above the line; the forms can be confusing.
• Fill in all information.  If there is something you can not fill in, such as automobile, etc, please write NA on that line.
• Make sure to sign on the bottom of the form

Cosigner Forms – Download

• Make sure the apartment address is written on both pages of the form.
• The forms must be filled out in their entirety.  If we do not have income numbers or SS numbers, we can not run credit and the property managers will not approve the applications.
• Original, Notarized forms MUST be mailed in as soon as possible.
