Write above the line; the forms can be confusing.
Fill in all information. If there is something you can not fill in, such as automobile, etc., please write N/A on that line.
We often need information such as a social security number to run a credit check, per the request of the landlord. If you prefer to call us and give that information over the phone instead, feel free to do so with your agent.
Make sure to sign on the bottom of the form and acknowledge that our brokerage fee was disclosed to you.

Make sure the “Apartment Address” and “Cosigner For” fields are filled out.
The form must be filled out in its entirety. If we do not have income or social security numbers, we cannot run credit and the property managers or landlords will not approve the applications.
Original notarized forms MUST be mailed in as soon as possible once the application is approved. Proper Realty Group is pleased to offer the services of a free on-site public notary at our 1340A Commonwealth Avenue location between the hours of 10 AM and 5 PM.

At Proper Realty Group, we strive to provide you with the highest value in your transactions, each and every time. The more money we can save you, the happier we are.
Brokerage fees are what we get paid for helping connect a landlord with a tenant. Often, our affiliate landlords are glad to pay the brokerage fee and you will be able to move in to an apartment with no additional costs besides the upfront rent costs and the costs of checking credit.
Some landlords are hesitant to pay brokerage fees for a variety of reasons. Many believe that their properties have such a high demand that a tenant will be willing to compensate the rental agent for their services in locating and securing the property.
You will always know whether a landlord is willing to pay a brokerage fee or not. If you love the property that you see with one of our agents, that agent will try and save you money by asking the landlord to compensate him or her. You are never obligated to pay anything until you accept the asking price or offer from the property owner and then choose to rent a property, so enjoy seeing places with our expert agents! W9Fill Out W9
If your rental agreement includes a security deposit, the landlord will need a W9 from one of the tenants to properly store the deposit in escrow. In fields 5-7, please enter the address of the apartment that you will be renting, not your current address.
